Through our independence management and professional nursing programs, we provide the support you need, from a half-hour daily visit to around-the-clock service. Our care planning is tailored to each individual and constantly adjusted, in consultation, to meet your changing needs.

The Rehab Care Management Program

OBJECTIVE:
To arrange, organize, or deliver cost-effective recovery-related care to insurance clients through a variety of venues. this co-ordinated process is designed to save funds by accelerating the patient's recovery.
SERVICES:
The focus of the program is to help patients return to normal, everyday bodily functioning as quickly as possible. However, sometimes people feel overwhelmed by the prospect of all that is involved in healing. It is at this point that they need caring intervention to make the road to recovery as simple as possible. Therefore, the program is an action-oriented process to help the patient attend to the business of recovery.
The Care Manager
The Care Manager is a Registered Nurse who co-ordinates the delivery of care for the patient, including prescribed exercises; arranging appointments with doctors, specialists, OTs, PTs, and chiropractors; transportation needs and more. The program extensively uses community resources such as community rec centers, swimming pools, exercise equipment, and so on. Progress is closely monitored by the Care Manager and by frequent consultation with the health care professionals participating in each patient's case.
The program could be accessible to those who:
- are receiving compensation for lost wages
- need assistance with motivation in the process of recovery
- visit multiple health-care providers
- are in hospital, but could receive care at home
A timely and comprehensive assessment is the first step in helping the patient achieve an optimum level of functionality. This assessment, conducted by one of our specially trained RNs, is used as the foundation for an individualized care plan, and is discussed with the attending physician.
